Swelling in the feet, ankles, or legs occurs when fluid accumulates in body tissues, a condition called edema. A long day of standing, extended sitting, heat, pregnancy, certain medicines, or a salty meal can sometimes produce temporary swelling. Persistent, worsening, painful, or sudden swelling requires more attention because causes range from vein problems to heart, kidney, liver, or clotting disorders.
Venous insufficiency is a common explanation. Vein valves normally help blood travel upward toward the heart. When they weaken, blood can pool in the lower legs, producing heaviness, aching, visible veins, and ankle swelling that becomes worse later in the day. Varicose veins are one visible sign of that impaired return.
Sodium can encourage the body to retain fluid. Processed foods, canned soups, sauces, snacks, and restaurant meals may contain more salt than expected. Reducing sodium may help some people with mild swelling, but anyone with heart or kidney disease should follow individualized instructions about salt and fluids rather than making major changes independently.
Swelling in both legs can accompany heart failure when the heart cannot move blood effectively and fluid builds in the body. Shortness of breath, rapid weight gain, unusual fatigue, coughing, or difficulty breathing while lying down make the situation more concerning. New or worsening swelling with breathing symptoms should be reported promptly.
Kidney and liver disorders can also disturb fluid balance. Changes in urination, puffiness around the eyes, abdominal swelling, yellowing of the skin, or marked fatigue may appear alongside leg edema. These signs require clinical assessment and laboratory testing because appearance alone cannot identify which organ, if any, is responsible.
Sudden one-sided swelling raises concern for deep vein thrombosis, a blood clot usually forming in a deep vein of the lower leg, thigh, or pelvis. The National Heart, Lung, and Blood Institute lists swelling, pain or tenderness, aching, warmth, and red or discolored skin among possible signs. A suspected clot needs prompt medical evaluation.
A clot can break loose and travel to the lungs, causing pulmonary embolism. Sudden shortness of breath, chest pain that may worsen with breathing, coughing blood, rapid breathing, faintness, or a fast heart rate are emergency symptoms. Call emergency services rather than attempting to treat them at home.
Injury, infection, surgery, lymphedema, and medication effects are other possibilities. Some blood pressure medicines, hormones, and anti-inflammatory drugs can contribute to swelling. Never stop a prescription abruptly. A clinician can review timing, dose, and alternatives safely.
For mild swelling with an obvious temporary cause, movement, avoiding prolonged sitting, and elevating the legs may help. Compression garments are useful for some vein conditions but are not appropriate for everyone, especially before circulation problems or a clot have been considered. Professional advice is important when swelling recurs.
Seek medical care when swelling lasts several days, keeps returning, affects one leg much more than the other, or comes with pain, warmth, redness, skin breakdown, or weight change. Severe shortness of breath, chest pain, fainting, confusion, or blue lips require immediate emergency help. Leg swelling is a symptom rather than a diagnosis, and its pattern determines how urgently the cause should be investigated.
A medical visit may include questions about when the swelling began, whether it improves overnight, recent travel or surgery, pregnancy, injuries, medicines, and heart, kidney, liver, or vein history. Examination of both legs and tests such as blood work, ultrasound, or heart assessment may be selected based on those findings.
Track useful details before the appointment. Note whether shoes become tight, whether pressing the skin leaves a dent, and whether swelling follows a particular meal, medicine, or period of inactivity. Do not massage a suddenly swollen painful leg when a clot is possible. Prompt assessment is safer than trying to force the fluid away.
